Project Summary

Every five years, local public health departments do a Community Health Assessment to see what areas are going well for community health and what could be improved. Information is gathered from population data, community organization surveys and focus groups. This information guides the next five years of health programming and efforts. Because of your feedback from the last Community Health Assessment, behavioral health was identified as a community priority resulting in the Broomfield Behavioral Health Improvement Plan.

Project Goals

The purpose of the Community Health Assessment (CHA) is to gather data to establish the health of the community, with the ultimate goal of creating a Community Health Improvement Plan (CHIP). A CHA is a process that identifies health needs and issues, while a CHIP is a plan that uses the results of the CHA to address those needs.

Public Engagement Summary

BPHE took a multi-faceted approach to gathering data and feedback, including a survey to community organizations and partners, a poll on Broomfield voice to prioritize issues that were identified in the CHA, focus groups with priority populations and engagement with stakeholders on strategy creation.

Health Equity Approach

Health equity is the ability for everyone to reach their full health potential, regardless of any socially determined status. Health equity in action involves removing barriers and providing resources for communities that have the least access to services and are most impacted by a health issue.

Broomfield Public Health and Environment (BPHE) is committed to promoting health equity in this process. Equity is not the same as equality. Information and feedback from certain Broomfield communities who have less access to health resources and more barriers to living well were prioritized.     Broomfield Public Health and Environment (BPHE), in partnership with stakeholders, identified strategies and developed the 2025-2029 Community Health Improvement Plan. The plan was presented to and approved by the Board of Health on Nov. 7. Individuals can review the Summary of the 2025-2029 Community Health Improvement Plan.

    In July, BPHE in partnership with a consultant conducted focus groups with priority populations and key community collaborators on the three priority areas: Mental Health and Substance Use, Housing Quality, and Healthcare Affordability and Preventative Care. These are the priorities for the 2025-2030 Community Health Improvement Plan. A report from these focus groups will be available in the coming weeks.

    On March 7, Broomfield’s Board of Health adopted Mental Health and Substance Use, Housing Quality, and Healthcare Affordability and Preventative Care as the priorities for the 2025-2030 Community Health Improvement Plan. Stay tuned on opportunities to engage around these priorities.
